INSTRUCTIONS FOR GENERAL DECLARATION FORM
AND SONORA / SINALOA DOCUMENTS

1. Our General Declaration form is meant to be opened and printed out on your computer's printer using Adobe Acrobat Reader. (See note below.)
   
   
   • Please click here to download our General Declaration form.
   • Print the form, using your computer's printer.
   • Fill out the printed form in blue or black ink.
   • Please send the completed form back to us, using regular mail. Also please send us:
     a) a copy of the issue page of your valid passport OR a copy of your birth certificate and
     b) a copy of your driver's license.
     Color photographs are no longer required.

   PLEASE NOTE:
   In order to view and print this General Declaration form, you must have Adobe Acrobat Reader installed on your system. If you do not have Adobe Acrobat Reader you may click on the following link: www.adobe.com and follow the instructions for installing the software. After the software is installed, you may close your browser window to return to this page.

   If you are unable to use our downloadable form, please let us know and we will send you the necessary forms through regular mail.
